Job Information
The Post holder will:  

·         Be competent as appropriate to the requirements of the role.

·         Demonstrate at all times the Core Values of client:

·         Holds current Pin Number with the NMC.

·          Has detailed professional and clinical knowledge. The practitioner will have experience in one or all of the following areas; Neurorehabilitation, Acquired brain injuries or Stroke rehabilitation with extended specialist knowledge related to specific patient needs.

·         Be skilled in the assessment of complex/specialist nursing required.

·         Able to provide care/support using the holistic care model throughout the hospital, ensuring continual evaluation of care to aid service development.

  • Be responsible for general running of the in-patient wards in the absence of the ward sister.
  • Be able to write detailed progress reports including Admission / discharge summaries
  • Attend MDT’s internal and external to the Hospital – presenting patients

·         Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance.

·         Demonstrate initiative and be creative in finding solutions to problems.

·         Work with the Ward sister and Senior Nurses undertaking delegated tasks in team management.



Nature of the Service 
The post holder will be part of the in patient service at the Hospital providing in-patient care and will support the patient in a number of settings, including community visits if required.

This is a dynamic post that requires the post holder to work closely with integrated care team including Consultants, Physio Therapists, Occupational Therapists and other multidisciplinary teams/agencies in ensuring seamless care and maximum rehabilitation across the spectrum of conditions treated at the Hospital.

Key Responsibilities/Duties     
1.       To adhere to and apply the Nursing & Midwifery Council (NMC) code of conduct, and all other relevant guidance from the NMC and follow our clients values. To be professionally accountable and responsible for all aspects of own work.


2.       To establish and maintain effective working relationships both within the Hospital and with external organisations in order to meet service requirements and support and demonstrate the Hospitals equality values.


3.       To assess patients and care/support needs. Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers. The specialist care/support plans may include the following:

·         Promoting independence/self management and maintaining safety

·         Increased functional ability

·         Improvement in health and wellbeing including mental health

·         Interventions in disease/condition management

·         Prevention and reducing of health inequalities

·         Supported discharge liaising with case manager and medical teams

·         Formulate Progress reports, treatment /care plans working a long side integrated care team.

4.        To consider each patient’s personal circumstances and preferences in order to implement and adjust programmes of care and intervention.
5.       To maintain and develop skills and knowledge to deliver a high standard of care and set an example of good practice to other team members including skill sharing.
6.       To maintain sensitivity at all times to the emotional needs of patients and their carers offering reassurance and empathy, in particular when imparting potentially distressing information.
7.       To use professional knowledge, skills and expertise when working in changing environments and across professional boundaries.
8.       To maintain contemporaneous and accurate records as required by our client and in line with professional standards (eg NMC). Support identified team members giving guidance and direction where appropriate.
9.       To ensure that the skills of identified team members are appropriately deployed and that care is delivered by staff with the relevant education, expertise and competence.
10.   To deliver a high standard of intervention, whilst managing the demands of unplanned aspects of the role using innovative problem solving techniques.
11.   To provide advice, education and training to patients, carers and professionals regarding the management and care/support of patients.
12.   To provide professional and clinical support and leadership to identified team members. Supports in  identifying training needs of staff and encouraging practitioners to develop skills in:

• Critical thinking and reflective practice

• Excellence in practice

• Specialist knowledge and research based practice

13.   To take accountability for allocated assessments, planning and implementation of care / programmes of support.


Performance and Quality
1.       To collect and input statistical information as required within agreed timescales.
2.       To participate in and support the team in undertaking clinical audit.
3.       To contribute to, and actively promote the implementation, development and upholding of policies, procedures, guidelines and directives, along with relevant statutory measures and governmental directives.
4.       The post holder may be required to order stock or sign off expenses for staff.
